COMPLETING CPF FORMS
Please be informed that the closure of CPF account and withdrawal of contributions is not processed in Australia.

There are two options to complete the CPF Account Closure form from Australia, and three options to submit the Closure form.
Completion Option 1: NOTARY PUBLIC AND APOSTILLE
- Have a Notary Public witness you sign Page 4 of the CPF Account Closure form at www.cpf.gov.sg/member/tools-and-services/forms-e-applications.
- Please note that the Notary Public does not need to witness you sign the CPF Account Closure form if you are able to submit the documents using your SingPass account or online through CPF’s website. Please see Submission Options 1 and 2 below for more information.
- Have a Notary Public provide a certified true copy of the required supporting documents. You can find the list of supporting documents on Page 5 of the Closure form. Please note you only need to provide one of each type of document required.
- After the Notary Public completes their part, you will need to send the CPF Account Closure form and the certified true copies of your supporting documents to the nearest DFAT Passport Office for them to Apostille the documents and authenticate the Notary Public.

Completion Option 2: IN PERSON AT THE SINGAPORE HIGH COMMISSION IN CANBERRA
Under CPF requirements, you may choose to complete and submit the CPF account closure and withdrawal application at any Singapore Missions abroad in-person. Submission by post or courier or witnessing via a video call is not allowed.
You will need to make an appointment. We have availabilities most mornings and afternoons, Monday to Friday (excepting ACT public holidays) until booked out.
The High Commission now utilises an online booking system for appointments.

What you will need to bring is:-
- the original form, completed but unsigned. Please see the form attached;
- your valid foreign passport;
- any supporting documents as required by CPF. The required documents differ from person to person. Please consult the CPF form for which documents apply to you;
- photocopies of those documents; and
- exact cash or an Australian money order (payable to ‘Singapore High Commission Canberra’) to cover the fee. The fee is AUD$21 per form and AUD$6 per page certified true copy.
If you are not able to photocopy the supporting documents, we can do that for you, but we will charge a fee of AUD$1 per photocopy.
Once the form has been completed and you have had photocopies of your supporting documents certified true copy, please proceed to submit the documents using one of the submission options below:
Submission Option 1: SINGPASS
If you have a valid Singpass, you can submit your application form and supporting documents via “My Mailbox”.
- Login to My Mailbox using your Singpass and 2FA-authentication.
- Click on +New Enquiry.
- Select subject Withdrawals due to Death / Leaving Singapore, and category Appeal – Withdrawal on Ground of Leaving Singapore and West Malaysia Permanently.
- Upload your completed application form (Form CPF-CA) and required supporting documents as stated under Important Notes on Page 5 of the application form.
- Have your supporting documents certified true copy by a local Notary Public, and then have the Notary Public authenticated by an Official at your nearest Australian Passport Office with an Apostille.
- You do not need to complete section 4 of the application form (“Witness Certification”).
- Note: if the file sizes are too large, you may need to submit using separate enquiries.
- Click Submit.
Submission Option 2: CPF WEBSITE
If you do not have a valid Singpass, but have a personal POSB, DBS, OCBC or UOB account maintained under your Singapore NRIC you can submit your application form and supporting documents via CPF’s website.
Please proceed to:
- Go to Write to Us.
- Select from A CPF member.
- Select subject Withdrawals due to Death / Leaving Singapore, and category Appeal – Withdrawal on Ground of Leaving Singapore and West Malaysia Permanently.
- Upload your completed application form (Form CPF-CA) and required supporting documents as stated under Important Notes on Page 5 of the application form:
- At section 2 of the application form (“Bank Details and Certification”), fill in the details of this bank account.
- Have your supporting documents certified true copy by a local Notary Public, and then have the Notary Public authenticated by an Official at your nearest Australian Passport Office with an Apostille.
- You do not need to complete section 4 of the application form (“Witness Certification”).
- Note: if the file sizes are too large, you may need to submit using separate enquiries.
- Fill in your personal details.
- Click Submit.
Submission Option 3: VIA THE POST
Please post the completed CPF Account Closure form, and the required supporting documents to:
