INVITATION TO QUOTE FOR PRE-EMPLOYMENT CHECKUP FOR EMPLOYEES
1) Specifications
The Consulate-General of the Republic of Singapore in Hong Kong would like to invite your company to quote for the pre-employment checkup for employees of the Singapore Consulate-General. The specifications are as follows:
(a) Pre-employment checkup which includes physical examination, blood pressure examination, chest X-ray, eye test and urine test and medical report (format of report should follow our sample).
(b) Pre-employment checkup (aged over 50) which includes physical examination, blood pressure examination, chest X-ray, electrocardiogram (ECG), eye test and urine test and medical report (format of report should follow our sample).
All rates and prices quoted should be exclusive of tax. If any tax is payable, it should be stated separately. Any enquiries regarding specifications, etc may be made to Ms Liew Pei Shan at Tel No: 2527 2212.
2) Closing Date
All quotations must be email to Hong_Kong_procurement@mfa.sg or by fax 2886 1239 no later than 3.00pm on 31 July 2015. Late submissions will be disqualified.
3) Validity of Quotation
The submitted quotations must be valid for a period of 90 days from date of submission.
4)Evaluation Criteria
Your bid will be evaluated based on the following criteria:
(a) Compliance with specifications;
(b) Track record of company; and
(c) Price Competitiveness
5) Award of Quotation
The acceptance and award of quotation will be at the sole discretion of the Consulate-General of the Republic of Singapore in Hong Kong and it will be posted on our website. The Consulate-General does not bind itself to accept the lowest quotation, or the whole or any part of any offer.
